No.  You can fill it out beforehand, but you may also fill out the application here at the Library.

For children age 15 and under, both parents (or the child’s legal guardians) must appear in person with the child.  You will need to bring:  evidence of the child’s US citizenship, such as an original birth certificate; and parents’ ID.  You will need to know the child’s social security number. Please visit www.travel.state.gov and go to “Passports for Minors” for more information on children’s passports.

     Payable in two checks or money orders of $130 and $35.
